As your dental records are currently held by BOH Dental, we need your consent to transfer these to to another practice within the Abano network of your choice prior to your next visit. This will mean that your dentist will have access to all of your existing records before you come in.
I authorise the release of my dental treatment records including copies of all relevant x-rays to be transferred to Maven Dental Group or 1300SMILES Dentists.
