How to claim $50 cash back on teeth whitening

1. Schedule a consultation with your local 1300SMLES Dentist that offers Philips Zoom in-chair tooth whitening treatment.
2. After completion and payment of treatment, you will be provided with a Zoom Receipt.
3. Visit www.philips.com.au/zoomcashback, follow the steps, and submit your claim online.

Philips will provide the cash back via Electronic Funds Transfer (‘EFT”) to the bank account nominated on the claim form. Transfer of funds will be made within 3-4 weeks of receiving the claim.

What is in-chair whitening?
