1300SMILES Dentists is giving local grassroots clubs the opportunity to receive up to $1000 through our 2022 Mouthguard Program! Complete the registration form below to nominate your club for the chance to be selected for the program.

Players from selected clubs will be able to purchase custom fitted sports mouthguards for just $175 at participating 1300SMILES Dentists practices. Each mouthguard purchased will earn the club $10, with a total of $1000 up for grabs per club.
